measurement Questions

We are using two Separate bug tracking tools (one for the core product and one for customisations). We want to merge them together because

1) we are finding that our customers need fixes before our core dept gets them merged into the core product

2) many bugs are fixed in the core product and our customers have not or choose not to upgrade. Often they don't know which build has the fix.

3) there is duplicate effort in both departments

Both teams are using Scrum 

Our team is looking to implement new metrics and I'm a fan of capturing DRE; however, I'm curious how others are calculating DRE within their organizations?  Specifically, when implementing this metric for an existing project that has already had numerous releases and has a number of existing defects in backlog, some of which will end up pushed to future releases and then probably pushed again.

Application software deliverable get's delayed due to Developer's or Tester's in IT industries? 

Which are the valuable charts or process guidelines to measure this ?

The criteria can be used cross projects and help to identify gaps and address them.

Most documentation that I have located to date only cover defect types and triggers for in process software development, I'm looking for a list of Triggers typically used to classify pose release defects.
