Start with the Obvious—Because Often, It Isn’t

When agile teams struggle with delivery, we often search for complex root causes: technical debt, lack of commitment, unclear business goals. But sometimes, the dysfunction is hiding in plain sight. The foundation of the project, the basic agreements and roles, is either weak or missing.

One of the most overlooked sources of agile breakdown is the project kick-off itself. Teams jump into sprints with enthusiasm, but if responsibilities aren't defined, communication lines are unclear, or foundational rules like "definition of done" are vague or absent, that enthusiasm quickly turns into confusion.

What Does a Healthy Project Foundation Look Like?

Before diagnosing advanced agile problems, take a step back. Is the project even structurally sound?

Here’s a checklist I’ve learned to rely on before scanning for deeper issues:

  • Clear Kick-off: Was there an official alignment session?
  • Definition of Done & Ready: Does the team share a mutual understanding?
  • Responsibility Mapping: Are responsibilities per topic clear and acknowledged?
  • Communication Flow: Does every team member know whom to reach for what?
  • Tooling Setup: Is the team aligned on which tools to use and how?

Without these in place, agile practices become rituals instead of results.

Why the Foundation Matters More Than We Admit

It’s easy to assume that teams “should know this.” But assuming is dangerous. Even highly experienced teams can fail if the basics are neglected. I've seen talented developers miss sprint goals, not because they lacked skill or motivation, but because they simply didn’t know who to contact to clarify topics quickly.

When the foundation is weak, every agile ceremony becomes harder. Stand-ups turn into confused updates. Sprint reviews lack clarity. Retrospectives become repetitive. Dysfunction becomes invisible, and frustration grows silently.

A Real-World Breakdown—and Fix

On a recent project I had been consulting on, delivery at the end of each sprint consistently lagged. No matter how we adjusted velocity or refined user stories, something always slipped. At first glance, it seemed like a matter of overcommitment or weak estimation.

But when I sat down with the team, something surprising came up: no one actually knew who was responsible for what.

The Scrum Master had launched the project without assigning clear topical responsibilities. Developers were improvising communication. Testers were unsure who owned the defects. Dependencies were getting lost in Slack threads.

I gathered the team for a reset session. We mapped out the core delivery areas and assigned clear responsibility owners per topic—technical, testing, business logic, and dependencies. We documented the structure and integrated it into our daily stand-up ritual. Within three sprints, velocity stabilized, blockers decreased, and ownership improved visibly.

Invisible Problems Require Visible Fixes

Agile dysfunctions don’t always look dramatic. Sometimes they’re quiet. Sometimes they’re foundational. As coaches, Scrum Masters, or leaders, our job is to start with what’s “too basic to be broken,” because that’s often exactly where the cracks hide.

When agile delivery feels off but no one can explain why, ask the team: Who owns what? When did we define that? Do we all agree? That simple audit can reveal a world of hidden problems—and unlock a path forward.
